Responsibilities of our QMC Hospitalists

  • Coordinate all hospital services for each member
  • Anticipate post discharge needs and discharge planning
  • Conduct AM patient discharges to allow for proper coordination of post discharge care
  • Consultative services for surgical and subspecialty patients
  • Keen attention to patient care-taker needs
  • Collaborate with par specialists and ancillary providers
  • Facilitate transitions in care through clear communication
  • Help manage large, challenging populations and balance overworked medical staff, making them invaluable within the system
  • Reduce readmissions
